Senior Project • Before Graduation you will need to complete a Senior Project • Every senior will have an Adviser @ AMHS. • Two Outcomes: • Senior Portfolio • Either a Standard or Distinguished Portfolio • Senior Presentation • A 10-15 minute formal PowerPoint presentation to display/highlight the contents of your Portfolio.

  5. Title Page: Type Name/Grade/Date/Adviser. Print (color optional). All forms can be saved once you enter your information.

  6. PowerPoint: A template is provided as a guide, but you may create your own. Print Your PowerPoint. Use Handout Option (6 slides per page) Color optional. PowerPoint emailed or saved on advisers computer. District filter will not allow files over 10MB to be received. All forms can be saved once you enter your information.

  7. Portfolio Rubric: This is not an interactive form. Print. Write Name/Adviser. All forms can be saved once you enter your information.

  8. Project Plan (ToC): Clickable Check Boxes for you to verify and organize your portfolio. Print (color optional.) All forms can be saved once you enter your information.

  9. Learner Divider: This is not an interactive form. Print (color optional.) You may want to do this on thicker stock to make it easier to find in your portfolio. All forms can be saved once you enter your information.

  10. Box Sheet: Fill out either Basic, or College. Fill Class Name in correct Subject Box for classes passed, or any classes currently enrolled in: You may need to shorten class name. Any additional classes taken in a subject goes in electives boxes. Extra classes may be ignored. Check lower right box only for classes you have completed. Click Print Button (color optional.) All forms can be saved once you enter your information.

  11. Best Work: You must have three. Two must be from Senior Year. Best works may not be strictly academic papers. (Dance Recital, Club Soccer Game, Sheet Music/Lyrics) Fill out Form(s). You type Advisers name on statement line (I _Mr. Enz__ acknowl) Print (color optional.) Must be signed by Instructor/Coach... All forms can be saved once you enter your information.

  12. Autobiography: This is not a .pdf form. While a sample has been supplied, you may use a more appropriate style for you future path. If enrolled in a senior level English class at AMHS during the first semester you may receive further instructions. Print

  13. Academic History: No link for this on the website. Adviser will have your transcript after 1st Semester grades are posted. Other certificates should be included: Dept Student of the Month MOS Certification Honor Roll EOC, SAT, ACT Scores College Acceptance Letter

  14. Citizen Divider: This is not a interactive form. Print (color optional.) You may want to do this on thicker stock paper to make it easier to find in your portfolio. All forms can be saved once you enter your information.

  15. Volunteer Record: You must have 12+ hours. Fill out Form(s) One form for each different Group. Example: 6 hrs YMCA 3 hrs Church 4 hrs Elem Tutoring Print (color optional.) Must be signed by Supervisor. All forms can be saved once you enter your information.

  17. Resume: This is not a .pdf form. While a template has been supplied, you may use a more appropriate template for you future path. References should be asked in advance and each able to speak about different aspects of you. Print. All forms can be saved once you enter your information.

  18. Letter of Rec Form: Type in information on both pages. You are not required to have official letters of recommendation, but may also include them with your portfolio. Print (color optional.) All forms can be saved once you enter your information.

  19. Post HS Action Plan: There are four forms. Pick only one. Apprentice (Welding, Machinist…) Employment (Non Apprentice work) Higher Ed (Tech, Comm.
