    2. What is Housing Pathway? An independent housing brokerage, advice and information service to people with Learning disabilities, their families and carers. Assisting individuals to make future housing choices.

    3. Aims To help people plan practically for the future. To identify choice based housing options through assessment of housing and support needs. To provide information, advice and support To develop & sustain good relationships with voluntary and statutory agencies to ensure we provide the right housing & support options to people with learning disabilities. To assist people to access benefits and acquire grants relating to their accommodation and support.

    5. How Does The Pathway Work? Identify circle of support for individual Identify realistic choices Identify funding requirements Broker & secure access to housing & support package to sustain housing Support to make benefit & grant applications Exit strategy & handover to support provider

    7. September – December ‘04 51 active housing referrals to Housing Pathway 4 People have so far established their own home 65 enquiries for housing related information and advice 13 people are in the process of moving to their new home 1 Person is considering shared ownership

    8. Who Can Use The Pathway? People who have a learning disability and need support to plan practically for the future People who are looking to leave residential care for more independent living. Families and carers who need advice and information regarding housing options. People who are leaving school Professionals who need information on housing options Older carers who need information on legal matters surrounding housing and ongoing support.
