Employee Protocol for Responding to Suspected Identity Theft Cases

This flowchart outlines the steps an employee must take when they suspect identity theft has occurred. Upon notification, the Department Director must determine if identity theft has actually happened. If confirmed, the CQO will enter the case into the CTS system and will involve necessary stakeholders, including the CEO and relevant department directors, to ensure patient accounts are adjusted accordingly. Additionally, it addresses how to handle potential disclosure of patient information and emphasizes the importance of following existing policies and consulting legal counsel as needed.
