102 學年度實習調查說明會. 楊基銓 主任 師資培育中心 102.12.13. 實習意願調查. 於 103 年 1 月 24 日前繳交 「實習意願調查表」 自行找尋並接洽實習學校 實習學校需為 「全國教育實習資訊平臺」 網站上 『 機構意願彙整審核查詢 』 查詢的到的才行；若沒有，請實習學校上傳實習資料方可。 實習學校之實習科系必須與任教學科的專門課程名稱相符。 實習學校若為綜合高中，請附 「學分對照表」 及學分對照表內兩校的課程規劃。 確認實習學校是否為本校合作簽約學校

  1. 102學年度實習調查說明會 楊基銓 主任 師資培育中心 102.12.13

  2. 實習意願調查 • 於103年1月24日前繳交「實習意願調查表」 • 自行找尋並接洽實習學校 • 實習學校需為「全國教育實習資訊平臺」網站上『機構意願彙整審核查詢』查詢的到的才行；若沒有，請實習學校上傳實習資料方可。 • 實習學校之實習科系必須與任教學科的專門課程名稱相符。 • 實習學校若為綜合高中，請附「學分對照表」及學分對照表內兩校的課程規劃。 • 確認實習學校是否為本校合作簽約學校 • 若是：填寫「實習同意函」，並經實習學校蓋印後，於上述期限前交至中心；逾期者，視為延緩實習。上聯繳交實習學校，下聯請交回本中心。 • 若非：接洽學校後，於上述期限前告知中心，由中心發文至實習學校，簽署合作契約書及實習同意函。

  4. 無規定表格，請學生自行製作。

  5. 實習前注意事項 • 請檢視目前所修「教育課程」是否符合本校教育專業課程之學分規定。 • 請檢視登記「任教學科的專門課程」是否符合教育部新核定之各任教專門科目一覽表學分要求。 • 實習前繳交文件，請參閱「103年第一學期實習資格審核流程表」。
