  1. Radiation Safety Surveys Requirements, Requests and Suggestions

  2. Radiation Safety Surveys Radiation Safety Surveys are required for all ionizing radiation producing machines.

  3. Common Deficiencies with Submitted Surveys • Surveys are illegible • Registration and equipment identifiers don’t match information on file with Dept. • Diagrams missing key identifiers including operator position, exposure switch location, lead shielding if present; wall, floor,and ceiling construction. • Missing information about the survey instrument used (i.e. calibration date, probe information, etc)

  4. Bureau Policy for Deficient Surveys • If survey deemed incomplete: • returned to the qualified individual who performed the survey for correction and re-submittal within two weeks • If no response: • Letter sent to registrant explaining that the survey does not meet regulatory requirements and must be corrected within 30 days • Enforcement Action if still deficient
