Educate target population. • Post-implementation data collect: • HCP survey immediately following resuscitation effort • FM survey 60 days following event Distribute post education survey LEGEND FPC-Family Presence Committee FPDR-Family Presence During Resuscitation FF-Family Facilitator HCP-Healthcare Provider RTL-Resuscitation Team Leader FM-Family Member Analyze results using SPSS Evaluate effect of education on medical staff opinion. • Data collection to support process change • Resuscitation outcome data quarterly • HCP assessment to identify barriers to FPDR quarterly • FM satisfaction measures quarterly • Financial impact of implementation biannually Form a team of all stakeholders. Develop process and tools to support FPDR. Finalize inclusion and exclusion criteria.
