Mission Statement • Main function of Sacred Heart Centre Early Intervention Services is to empower children with a broad range of intellectual and physical challenges and their families to develop to their full potential. • This is achieved through the provision of an Individual Service Plan for children attending the Main EI service 'integrating care with care in partnership with families' Mary Lacey Crowe

What services do we provide ? • Provide daily intensive intervention to children with multiple complex needs Monday – Friday • Provide programme input to children attending the Toddler/Playgroup, currently 2 sessions per week • Provide day respite to children with complex needs when School is closed • Training/ support to families/community pre schools/ schools as appropriate 'integrating care with care in partnership with families' Mary Lacey Crowe

What service to access ? • Main Early Intervention service……..child is nearing 2 years of age….will have significant development delay/needs in a number of areas…will require a significant amount of individual attention and will only participate in small group situations for short periods of time • Will have access to various personal, social, cognitive, communicative, fine and gross motor programmes 'integrating care with care in partnership with families' Mary Lacey Crowe

Service….cont’d • Toddler Group – 18-36 months……..is compliant for most activities….able to participate in small group situation and has established joint attention… • Will have the opportunity to further develop important social, cognitive, language, motor and adaptive skills…will continue to attend therapy sessions outside programme time. • On discharge from the programme the networks will continue to lead out the necessary interventions in consultation with the family and others 'integrating care with care in partnership with families' Mary Lacey Crowe

Services cont’d….. • Play Group – 36 – 60+months……..will have established joint attention and is capable of participating in small group situations……will access various personal, social, cognitive, communicative, fine and gross motor programmes to further develop existing skills…….protocol same as Toddler Group 'integrating care with care in partnership with families' Mary Lacey Crowe

Prior to admission….. • The Clinical Nurse Manager meets with the parents and child within a therapeutic forum and the family will have completed The Sacred Heart Admission/Assessment Form which provides pertinent data on… • Family information/history….all people and supports involved • Birth and medical history • Developmental history • Digestion/elimination history • Sleep pattern • Oral hygiene • Risks ..that reflects a complete and accurate picture of the child’s strengths, needs, preferences and environment…and that addresses the family questions about enhancing their child’s development with particular focus on each family member’s concerns and priorities 'integrating care with care in partnership with families' Mary Lacey Crowe

Following on from this…. • Families are given an overview of the service including information on …… • Expectations • IFSP • Policies & Standard Operating Procedures • Consent • Health & Safety • Communication…customised Log Book….and finally meet the staff team 'integrating care with care in partnership with families' Mary Lacey Crowe

Individual Family Service Plan (IFSP)….at the Sacred Heart Centre…. • Documents and guides the early intervention process for the children who attend the main EI service and their families • Through this process, family members and staff work as a team to plan, implement and evaluate services tailored to the family’s unique concerns, priorities and resilience 'integrating care with care in partnership with families' Mary Lacey Crowe

IFSP cont’d… • Is written within 1 month of the child entering the service • Takes place bi-annually .. at a time suitable to the family- sent out 3/4 days in advance for parents to review/ check for accuracy and update their family goals 'integrating care with care in partnership with families' Mary Lacey Crowe
