  2. The challenge • Care of people with long term conditions accounts for: • 50% of all GP appointments • 64% of outpatient appointments • 70% of all inpatient bed days • There were 15.1m total hospital admissions in 2012/13, compared to 11.4m in 2002/03.

Technology enabled care services programme • Working collaboratively with stakeholders from industry, the NHS, social care, housing, the third sector, commissioners and providers, we have identified the issues and barriers to the wider adoption of technology enabled care services, highlighted the following four key areas as levers for change: • commissioning • procurement • measurement and metrics • information governance
