  2. What is HomePower? HomePower is a group adult foster care program created through a partnership between the Visiting Nurse Association of Boston and Boston Senior Home Care. HomePower provides daily in-home, personal care to eligible adults throughout the Greater Boston area.

  3. Did you know that HomePower remains active on a case even while a visiting nurse, therapist, or home health aide is actively providing skilled services? HomePower services can also be used in collaboration with hospice care.

  4. Personal Care Homemaking Alternating monthly home visits by a RN and Case Manager Personal Emergency Response System Adult Day Health Program Home Delivered Meals Grocery Shopping Laundry Money Management HomePower Services

Direct physical care: sponge bath, shower, foot-care, supervision with bathing help with dressing and grooming assistance with transfers and ambulation Other Services: preparing light meals & light housekeeping running occasional errands medication reminders (administration of medication not permitted) Personal Care Homemaking includes:

Care Managers create a comprehensive client-oriented Needs Assessment consisting of identifying current client unmet needs or problems that inhibit independent living within the community; assisting the client in one or more home care services; and developing and initiating a home care service plan for the client.Case management also includes monitoring and evaluating the client's home care plan in terms of its continued effectiveness in maintaining independence.   7. Must be at least 22 years old with no more than three people living in the household. Must have Standard MA Health or CommonHealth. Live in a managed building with 24 hour access. No private homes. Have functional or cognitive impairment that prevents them from living safely and independently at home. Does not need to be homebound, but must require and accept assistance with personal care 5-7 days a week. Who is Eligible for HomePower?
