IBD Pathway of Care 2 IBD – Stable or in remission IBD register Not in specialist follow up Specialist follow up Offer option of primary or secondary care review Primary care Secondary care Structured annual review based on image template Via referral, helpline or IBD nurse (5 working days) Practice responsible for confirming completion Relapse or indication for specialist review (depending on extent and severity – see MoM)
