Components ofSA Treatment System • Substance Abuse continuum of care comprises 9 major components: • Detoxification • Intensive treatment (Inpatient or Outpatient) • Residential Programs • Outpatient Services • Pharmacotherapy • Aftercare • Maintenance • Education and Psycho-Education • Adjunctive Services

Detoxification Programs • Medical, Hospital Detoxification • General hospital or special hospital unit • Must be admitted by a Physician • Medical, Non Hospital Detoxification • Community based / Social detox centers • Admission procedures vary • Length of stay in detox. programs depends upon substances being used and insurance providers

Detoxification • Medications administered most frequently for withdrawal include: • Librium for Alcohol • Phenobarbital for Barbiturate • Methadone or Clonidine for heroin • Effectiveness of Detoxification: • In terms of reducing subsequent drug-taking behavior and / or relapse, detox alone has no discernible effect

  7. Intensive Treatment • Intensive Inpatient Care: • Duration of stay varies • Relatively expensive • Difficult to get approval • Often have long waiting lists • Most programs are based on “Minnesota Model”, which is an abstinence oriented, comprehensive, multi-professional approach to treatment

Intensive Treatment (Cont.) • Intensive Outpatient Care: • Preferred method of Insurance providers due to reduced costs • Preferred method for: • Single parents • People with limited resources • Those who can’t afford to be away from their job for a long period of time • Others who can safely continue to function at home and in the community • Typical program runs three or four nights a week for two to twelve weeks

Intensive Treatment (Cont.) • Day Treatment: Another type of Intensive Outpatient Program • Generally lasts longer than other intensive treatment programs • Often used by clients with co-occurring disorders, physical impairments, or substantial cognitive impairments that require a longer period of rehabilitation • Services are similar to those provided by most inpatient programs

Residential Programs • Halfway Houses • A transitional place of indefinite residence • Residents must abstain from alcohol & drugs • Structure and services vary • Size and credentials of staff also varies • Many have resident or community government • Fees are usually relatively modest compared to other alternatives • A few are designed for women with children

Residential Programs (Cont.) • Therapeutic Communities (TC’s) • Designed to treat addicts who abused heroin or other illegal drugs • TC staff are often addicts with a substantial period of recovery • Generally provide a combination of intensive treatment and residential care • Lengths of stay range from a couple of months to two years • Tend to use high confrontation group process and peer pressure • Residents earn privileges as they progress

Residential Programs (Cont.) • Domiciliaries: • A type of residential facility designed to assist those with severe physical or mental debilitation resulting from substance abuse • Provides an opportunity for residents to achieve a substantial period of recovery before moving to a Halfway House • Usually have 24 hour staff supervision • Often referred to as farms • Care is mostly custodial with some group therapy

Residential Programs (Cont.) • Missions and Shelters: • Assist those who are homeless, transient, or living on the street • A few of them could be classified as Halfway Houses due to their relatively long 30 to 90 day periods of stay, but most are more like a shelter with “Three hots & a cot” • A religious program is generally a component of services. • Many require individuals to leave the premises after breakfast and don’t allow them to return until evening check in time

Outpatient Services • Various theoretical orientations and treatment philosophies • Frequency varies - Most provide weekly sessions • Content of sessions usually similar to those of intensive and residential treatment programs • Group therapy is usually “treatment of choice” • Individual therapy is used as an adjunct • Brief & very brief interventions are utilized

  27. Pharmacotherapy • Several types of drugs are used to assist alcoholics and addicts in pursuit of recovery • Disulfiram: (Antabuse) • Used as an adjunct deterrent to impulsive drinking • Interferes with normal metabolism of alcohol, resulting in a serious physical reaction when alcohol is ingested • Contraindicated for those with serious mental illness, heart disease, diabetes, epilepsy, pregnancy • Methadone: (Synthetic Narcotic) • In addition to its use for detoxification, methadone is used in long term dependency treatment as a substitute for narcotic analgesic drugs

Pharmacotherapy (Cont.) • Methadone: (Continued) • Although its use is controversial, and critics argue we are simply replacing one drug with another, it has been instrumental in reducing addicts illegal activities • Advantages of methadone over other forms of maintenance therapy are: • It can be administered orally • It acts on the body for 24 to 36 hours • It causes no serious side effects • At proper dose levels, it blocks the effects of heroin • When taken orally - No substantial euphoric effects • Disadvantage is that Dependency develops

Pharmacotherapy (Cont.) • LAAM: • Methadone analog with longer lasting effects • Advantages: • Taken three times a week • Equally as effective as methadone at: • Reducing heroin use • Decreasing arrests • Reducing treatment dropout • Helping to maintain employment • Disadvantages: • Appears to be more addictive than methadone with more severe withdrawal symptoms • Not approved for pregnant or nursing women

Pharmacotherapy (Cont.) • Buprenorphine: • An analgesic and opioid antagonist that seems to block the effects of opiates • Appears to only create low physical dependence and mild withdrawal symptoms • Clonidine: • An anti-hypertensive drug described as the first non-addictive, non-euphoriagenic prescription medication with demonstrated efficacy in relieving the effects of opiate withdrawal • Not for everyone - It lowers blood pressure

Pharmacotherapy (Cont.) • Naltrexone: • A synthetic narcotic antagonist that is used primarily for: • Treatment of narcotic overdose because it can reverse respiratory depression caused by narcotic drugs • Longer term treatment to block the euphoric effects of narcotics • It is reportedly not addictive and its effects last for a few days. It is best suited for: • Adolescent users with short term heroin use • Recently paroled prisoners • Helping people off of methadone maintenance

  35. Aftercare • An extension of intensive treatment, residential and outpatient services along the continuum of care • Helps prevent relapse by: • Providing ongoing support • Assisting clients in monitoring their progress and addressing the problems or obstacles they encounter in trying to maintain recovery

  36. Maintenance • Recovery is a lifelong process usually assisted through participation in 12 Step and other self-help groups • May require periodic return to short term aftercare or brief treatment when a crises occurs TIME FOR A BREAK

Education & Psychoeducation • A crucial element of all substance abuse treatment programs is didactic education • Psychoeducational groups combine presentations of didactic information with other techniques like: • Role plays • Structured exercises • Reading and Homework assignments • Keeping journals • Group discussions

  38. Recovery Support Services • A multi-dimensional approach requires assisting clients with resolving major problems that impact their lives like: • Employment • Legal issues • Family issues • Health • Spiritual • Other significant problems • A referral may be sufficient for some, but most will need some type of additional assistance • These services are usually coordinated through case management
