    3. Review the course offerings and put an (X) in the box next to your selections totaling (7) periods. Course Names in bold require a teacher’s signature for selection. To request a zero period, attach your special request form. One course you select must be the zero period course and another must be an extra course, totaling 8 periods You are guaranteed your academic selections but YOU ARE NOT GUARANTEED YOUR ELECTIVES or ZERO PERIOD CHOICES! Be sure to choose alternatives for your electives. If you don’t, they will be selected for you and SCHEDULE CHANGES WILL NOT BE AN OPTION IN THE FALL. Review this form with your parents and return this form to your guidance counselor with a parent signature on the due date. If your completed form is not returned by the due date, your schedule will be selected for you by the school without your or your parent’s input.
