Responsibilities ofHealth Officer or Designee • Assuring that Physician and RN are in compliance with this policy and procedure • Assuring competency of RN through initial training and annual review of training • Conducting an annual review of dispensing-related activities of the RN • Obtaining the approval of the Committee on Nurse Dispensing for each LHD’s formulary and any changes or additions to that formulary

Responsibilities ofHealth Officer or Designee • Obtaining the approval of the Committee on Nurse Dispensing for each designated trainer used by LHD • Authorizing RN to dispense (completion of training and delegation from authorized physician) • Allowing that the Division of Drug Control enters and inspects at reasonable hours • Reviewing the training curriculum

  4. Responsibilities ofHealth Officer or Designee • Assuring written authorization or release of information is signed by patient or patient’s authorized agent • Assuring safeguards are in place to maintain the confidentiality of patient’s drug records

  5. Responsibilities ofHealth Officer or Designee • Restricting access to patient’s drug records to only: -Adult patient -Emancipated minor -Unemancipated minor when consent was sufficient to authorize treatment -Parent or legal guardian -Prescriber -Licensed practitioner -Pharmacist

Responsibilities ofHealth Officer or Designee • Restricting access to patient’s drug records to only: -Person who signed Authorization for Release of Information -Person authorized by subpoena, court order or statute -Third person who is paying medical expenses -Board of Pharmacy/Division of Drug Control -Executor, spouse or administrator of a deceased patient -Approved researchers

  7. Responsibilities ofHealth Officer or Designee • Assuring that no additional fee is charged for dispensing drugs or devices • Assuring that a licensed pharmacist is available for consultation • Assuring that up-to-date drug references are available to RN’s
