  2. Innovation Fund • Funded by CCG through Wyre Forest District Council • Bids assessed quarterly by DC officers, CCG rep and Public Health • Agreed by Strategic Health and Inequalities Partnership / CCG • Support priorities of CCG and Health and Wellbeing Board

Innovation Fund Process; • CVS can apply for innovation funding via the CCG website. • Complete and application form that comes before a panel • Bids are evaluated against a set criteria including value for money and meeting the objectives of the HWB / CCG / DC. Grant conditions; • May be requirements to meet specific need or location • Evaluation / Monitoring reports and case studies to evidence benefits

Innovation Fund Proposals for 2014/15; • Mental health (using learning from pilots/pathway work) • Dealing with debt and the impact on health and well-being (now the Anti-Poverty Strategy); • Tackling obesity including through extending the food programmes and enhancing walking schemes (+ other physical activities) • Health Inequalities (particularly in the 0 – 5 age group) • Long Term Conditions / Older People (social isolation)
