  4. Fracture neck of femur • Commonest site in elderly • 6th and 7th decade • Associated with osteoporosis • Other factors – osteomalacia, stroke, diabetes, alcoholism, chronic disease • Post menoapause

  5. mechanism • Simple fall , twist/ external rotation of leg in elderly • In young individuals, fall from height

  7. Clinical features • History of fall • Pain in the hip • Limb is shortened and in external rotation • In impacted fracture – patient may walk • In young patient- may be associated with multiple fracture

  8. investigations • X-ray • CT scan – if associated with pelvic fracture

  9. Diagnosis • Stress fracture • undisplaced fracture • painless fracture • Multiple fracture

  10. Treatment options • Conservative • surgical

  11. treatment • Young individuals :- surgical emergency • Operated within 12 hours

  12. Internal fixation • Reduction and cancellous screw insertion

  14. treatment • Elderly patients with displaced fracture

  15. complications General :- • DVT • Pulmonary embolism • Pnemonia • Bed sores

  16. complications • avascular necrosis • Non union • Osteo arthritis

  17. Associated fractures • Associated with fracture shaft • 20%
