Patient Safety Learning Networks (PSLN) Project Overview (2011-13) • Purpose: Recruit and support hospital teams to learn how to use AHRQ-funded tools and resources to improve patient safety • Tools and Resources Offered • Hospital Consumer Assessment of Healthcare Providers and Systems (using HCAHPS to measure and improve the patient experience of care, and reduce readmissions) • Medication Reconciliation toolkit (reduce ADEs and readmissions) • Project RED (to reduce readmissions) • Door to Doc (improve ED flow management) • Quality Indicators (identify hospital adverse events) • PSLNs sought to create ongoing collaborative relationships between hospitals
