  3. Scholarship Committee • Volunteers from BOD (no educators) – Review Applications and Announce Winners, Verify Enrollment – July-Sept • October BOD meeting • $500 for fall semester/$500 for spring semester (once enrollment verified) Awarding of Money

  4. Connecticut Society for Respiratory Care Scholarship Application – 2010 *This application is for a new/incoming student enrolled in an Accredited Respiratory Care Program at a Connecticut college. SCHOLARSHIP ELIGIBILITY: The applicant must submit the following information and meet the listed criteria for eligibility. If ALL documents are not submitted with the completed application, the application will not be considered. 1. have an overall GPA of at least 2.75, or the equivalent thereof, as verified by submission of an unofficial transcript signed by the Guidance Counselor/Advisor from the school in which you are currently enrolled 2. be a U.S. citizen and submit verification copy of a birth certificate or US Passport 3. essay paper in response to one of the two questions contained within this application 4. letter of recommendation from a faculty member of the college or high school in which you are currently enrolled 5. submit this application and all of the required documentation by July 1, 2010.

Required Essays New/incoming Student • 1) What life events, personal encounters, or experiences helped you decide to become a Respiratory Therapist and why? • 2) What qualities or attributes do you possess and plan to bring to the respiratory care profession that will help you succeed as a Respiratory Therapist?

  6. CURRENT STUDENT • 1.) Describe a clinical experience you had while attending the respiratory care program that solidified your decision to pursue respiratory care as a career. • 2.) Respiratory care is a relatively unknown allied health profession. As a future respiratory care practitioner, describe what you would do to increase public awareness of the profession.

  8. How Do We Fund the Scholarships? • Basket Donations at Annual Symposium • Done by individual departments • Each basket is themed • CTSRC “stuff” for Sale at Annual Symposium • Speaker Donations • Golf Tournament • Individual Department Activities during Respiratory Care Week
