Submission Requirements • HSAG requested the following information: • 2011 HEDIS Roadmap • 2011 Final Audit Report • 2011 Final locked IDSS file (.csv and .xls format) • For those MCOs who do not report to NCQA, submission of the 2011 AHCA Reporting Template • Supplemental Questionnaire: HMOs/PSNs If you have both a reform and non-reform contract and the process to collect and calculate the performance measures are the same, you may submit one document and indicate that the submissions addresses both contracts.
