Why an INEOS Wellness Program? • Healthy employees are an important element of a strong organization • Encourages employees and spouses to become more aware of own current health and health risks • Annual testing and early detection will help greatly to prevent or mitigate future illness • Awareness drives change, change drives improvement • Critical step in keeping health care costs in-check, aimed at slowing the rapid rise in health care premiums that will benefit us all

  3. Eligibility • Who is eligible to participate? • All employees enrolled in an INEOS Health Plan • Spouses/domestic partners enrolled in an INEOS Health Plan • COBRA and Retirees • Participation is voluntary • You both do NOT have to participate • Children are not eligible to participate

  4. Participation Criteria • Complete a routine annual physical • Complete age- and gender-appropriate blood screen including glucose and lipids testing • Complete the BCBS Health Risk Assessment (HRA) Note: You must complete all tasks to receive credit under the program

  5. Health Care Account • If you complete the criteria, you receive $200 credit in a Health Care Account (HCA) at BCBS • If your spouse/domestic partner completes the criteria, he/she will receive a $200 credit in a Health Care Account (HCA) at BCBS • Spouse/domestic partner accounts are separate from employee accounts

  6. Health Care Account • Health Care Accounts can be viewed online at www.bcbsil.com under BlueAccess for Members (BAM) • Registration is required • What you will see • Deposits • Balance • Claim activity

Health Care Account • Your HCA can be used to pay for: • Deductibles • Coinsurance • Copays • Your HCA cannot be used to pay for: • Dependent expenses under the plan if you have family coverage or employee + child(ren) coverage • Prescription Drugs • Any expenses that are paid outside of the plan Note: You cannot use your HCA to pay for spouse/domestic partner’s expenses (or vice versa) even if he/she participates in the program

  8. Health Care Account • Unused account balances rollover to the next year • You lose your HCA when you cease participating in an INEOS Health Plan

Costs for Participating • No cost to you for participating • All health plans were changed effective 7/1/2010 to cover wellness visits at 100% • Employees and/or spouses/domestic partners that have completed the criteria will receive reimbursement for any out-of-pocket wellness related expenses through the HCA

  10. Annual Physicals • If you completed an annual physical since 10/1/2009, you will not have to repeat this task • If you completed a company physical (including blood screen) and wish to have this count towards meeting this criteria, you will need to complete a Wellness Release Form (available from your Occupational Nurse/Doctor or HR). • Wellness Release Form allows your name to be released to the Benefits Department indicating that you’ve completed this task. • Annual OB-GYN and PSA physicals count towards completing this criteria

  11. Age- and Gender-Appropriate Screenings • If you completed a physical but did not have the appropriate blood screens performed (lipid and glucose), to qualify you must complete this task. • If you do not have the results of these tests, you should obtain a copy from your doctor before completing your HRA.

  12. Health Risk Assessment • The HRA can be found on the BCBS website under the Personal Health Manager link under My Health • What information is needed: • Blood screen results • Blood pressure • Weight/Height • Takes about 15 minutes to complete

  13. Privacy • No Personal Health Information will be shared with INEOS • Your rights are protected under the Health Insurance Portability and Accountability Act of 1996 (HIPAA) • Only aggregate reporting to INEOS

  14. New Enrollments 1/1/2011 • If you newly enroll in an INEOS Health Plan 1/1/2011, you may participate in the Wellness Program for plan year 1/1/2012 • No HCA credit will be given until 1/1/2012

  15. Next Steps • What you need to do: • Complete annual physical • Complete blood screenings • Complete HRA Note: Tasks must be completed by 9/30/2010 • Watch for letter from the Benefits Department during November 2010 confirming your participation • On or about January 1, 2011, verify your HCA account was established
