Summary • From an Operator’s perspective, we have to understand “WHY” events occur and then put “DEFENSES” in place to mitigate error-rate • In “Emergency Situations” we have to recognize the error-precursors that challenge Operators the most • Using our Training Simulators to teach Operators how to address emergencies from a team perspective is key to surviving challenging events
