Coleman Transitional Housing Facility for Young Adults • NEED AND OUTCOMES • The funds were used to purchase and rehabilitate the entire foreclosed structure from basement to attic creating a transitional housing facility for homeless young adults’ ages 18 to 26 who are either exiting the foster care system, a mental health or drug addiction program or a correctional institution. • Coleman Professional Services is following the “Youth Model of Care” in operating this facility. One full and one part-time staff member employed at the facility shall attend TIP (Transition to Independence Process) training soon to implement this evidence based practice of care.

Coleman Transitional Housing Facility for Young Adults • NEED AND OUTCOMES (Cont.) • Coleman anticipates that after the approximate 12 to 24 month period of stay at least fifty-percent of the homeless young adults will have secured a source of income through employment or benefits and permanent housing. The house is now the “nicest” house on the street; one of the goals of the Neighborhood Stabilization Program when it was first introduced to Trumbull County in 2009.

Coleman Transitional Housing Facility for Young Adults • IMPACT TO COMMUNITY • Reducing the number of blighted and foreclosed properties in the City of Warren is a top priority for all stakeholders in addition to meeting the goals specified in Trumbull County’s Blueprint To End Homelessness Plan drafted a few years ago through the Trumbull County Housing Collaborative Continuum of Care Group. The newly organized regional mental health board is looking at this facility as a model for creating facilities just like it throughout Ohio.
