HEADS Model • H= home • E= education, employment, eating • A= activities • takes in interests and friendships • D=drug usage • cigarettes, alcohol, marijuana, other drugs; using and driving = depression • mood, suicidality • S=sexuality • sexual orientation, sexual activity and safety • may be where issues of gender identity emerge as well = general safety • home, school, neighborhood, weapons at school, guns at home, use of seat belts

  3. SHQ and HEADSMiddle School HOME/SCHOOL • 1. Who do you live with? • 2. Who do you feel you can really talk to? • 3. Are you having any of the following issues at home or school? PHYSICAL HEALTH • 4. Do you usually participate in physical activities, such as walking, skateboarding, dancing, swimming, or playing basketball, for a total of 1 hour every day? • 5. Do you usually watch TV, play video games, or spend time on the computer for more than 2 hours per day (not including computer time for school)? • 6. Do you usually eat 5 or more servings of vegetables and fruits every day? • 7. Do you usually get 8 or more hours of sleep every night? • 8. In the last 12 months, have you seen a dentist or gone to a dental clinic?

Do you always wear a helmet when rollerblading, biking, motorcycling, skateboarding, ATV, skiing or snowboarding? • 11. Is there someone at home, school, or anywhere else who has made you feel afraid, threatened you, or hurt you? • 12. Has a boyfriend/girlfriend ever hit, slapped or hurt you on purpose? • 13. Have you ever carried a weapon (gun, knife, club, etc.) to protect yourself? • 14. Have you ever been in foster care, a group home, homeless, or had to live with another family member or friend? • 15. Have you ever been in jail or in a detention center?

Are you often tense, stressed out, and/or have difficulty relaxing? • 18. Over the past 2 weeks, have you noticed feeling down, depressed, irritable or hopeless? • 19. Over the past 2 weeks, have you noticed less enjoyment or interest in doing things?

Are you, or do you ever wonder if you are gay, lesbian, bisexual, or transgender? • 25. Have you ever had sex (including oral sex)? • If you answered “Yes” to question 25, please complete questions a-c a) Do you and your partner(s) always use condoms when you have sex b) Are you using a method to prevent pregnancy? Which types c) Have you ever been pregnant or gotten someone pregnant?

Moving Beyond Ourselves Important Reminders: • recognize and be conscientious of your biases • Acceptance of difference of knowledge bases about health behaviors and health causality • Relinquish Control- allow for self-awareness through the conversation • Willingness to negotiate imperfect accommodations and bridge the differences

Useful Tools • Use both verbal/non verbal cues • Interact in a non-judgmental way (or what can be perceived as so) • Open-ended questions that allow the patient and provider to have an open line of communication • Listen for what is not being said • Use your negotiations skills • Be considerate of cultural differences and needs

HEADS Model Essential elements when interacting with adolescence: H= home E= education, employment, eating; A= activities D= drug usage/depression (mood, suicidality) S= sexuality & general safety • Chart Review for SHQ/Permission Slips • Statement of Confidentiality • Create Rapport • Use Patients Name • Recognized Verbal & Body Language • Use Open-Ended ?’s • Active Listening • Use Non-Judgmental ?’s/Statement (Know Ur Biases) • Remember No Assumptions • Provide Health Material • Understandable Terminology • Follow-Up (As Needed) • Exercise Warm Handoff (As Needed)
