  3. Resources • Same care pathway for referrals • CPN identified • Divert resources from CMHT • Redirected to homes • Advertised and promoted • Improved links with GPs/Homes • Pilot launched

  5. Reason for Referral to Link Nurse Jun – Dec 2010 Jan – Jun 2011

  6. Interventions • Initial Holistic Assessment • Implementation of Behavioural Charts - Strategies to manage behaviour • Medication Review • Placement Review • Advice & Discharge

  7. Outcomes • Very positive impact on use of hospital beds/large decrease • Results evaluated show 88% admission avoidance • 98% through behavioural management • Teaching management techniques • Small percentage with medication • Homes/GPs report positively on response • Homes more comfortable in seeking help at an early stage.

  8. Lessons learnt • Clients need to be referred quicker • Direct and timely interventions prevent admissions • Outreaching to homes and GPs reduces admissions • Education and understanding dementia management reduces admissions • No reduction in CMHT capacity as resources were redirected
