  1. Genital Herpes

  2. 1、Definition Genital herpes is a sexually transmitted disease (STD) which is main to be caused by the herpes simplex viruses type 2 (HSV-2).HSV can cause blister、erosion and sores, which appear in and around the skin or mucous membranes of the genital area.

  3. 2、etiology The pathogen is herpes simplex viruses (HSV), HSV- type 2, 90%。 HSV- type 1,10%。

  4. 3、transmission and epidemic • way of infection:The sex contact infection is the main path of the GH。 • source of infection:the patients and nonsymptomaticvirus carrier of GH. • mode of infection: HSV-1 is often transmitted by droplet and saliva,HSV-2 is chiefly transmitted by sex contact . • Genital herpes is one of the most common sexually-transmittedviral diseases in Euro-American • The incidence of GH is higher in the infected of HIV.

  5. 4、clinical manifestation • 1. Primary genital herpes: • The incubation period is 2 to 20 days. • The eruption starts with erythema and papule that develop rapidly into crops of vesicles, produce erosion after vesicles rupture, and ulcers can be resulted from secondary infection. • The disorder is characterized by incandescence, malaise, and pain when ulcers occur. • The disorder lasts between 1-2 weeks。

  6. 2. recurrent genital herpes: The herpes simplex viruses is transported to sacral ganglia via peripheral nervous and incubate for long-term. The recurrences of GH are triggered by trauma、fatigue、infection、menstrual onset or cold and so on ,while the viruses travel along the nerve paths to the surface of the skin and mucous membranes which are often involved. The signs and symptoms of recurrences are mild.

  7. Researches of recent years report that HSV-Ⅱ is associated with the development of carcinoma cervicis, there are the evidences: • Women with GH correlate with a higher incidence of cervical cancer • women with GH have higher sero-positive for HSV-Ⅱ antibody • HSV-Ⅱ antigen found in the carcinoma cervicis cells • The DNA of HSV-Ⅱ cultured in histiocyte can transform the hamster cells into cancer cells.

  8. 5、diagnosis and differential diagnosis • GH shoud be differentiated from other diseases with erosion and ulcer in the genital area:: • chancre • chancroid • Behcet’s syndrome • ulcus vulvae acutum

  9. 6、treatment

  10. topical treatment: The local shoud notice to be clearing and drying, use 5% acyclovir ointment and 3%phthiobuzone cream topically.

  11. antiviral therapy: acyclovir 0.2g q4h,5 times daily for 7~10 days

  12. immunomodulator: Levamisole, interferon, transfer factor、poly I:C、Chinese traditional and herbal drugs and so on.
