  2. Application Submission • Applicants are required to apply online and submit hard copies to the CDE. • Applications must be received by the CDE no later than 5:00 p.m. on November 30, 2012. • No late submissions will be accepted.

  3. Online Application Process • The After School Support and Information SysTem (ASSIST) is an online application system that will collect all necessary information for application submission and create all necessary application forms and assurances. • A secure logon must be obtained prior to using the online ASSIST system. Please follow the instructions for one of the three options described in the ASSIST Application Instructions document posted with the Request for Applications document. • Once the logon is obtained, go to the CDE ASSIST Grant Management Web page at http://www3.cde.ca.gov/assist/index.aspx.

Final Review Each grant application must consist of an electronic submission via ASSIST, an application narrative, and a downloadable ASSIST forms packet that includes the following: • Cover Page • Additional Required Information • Signatures and Approvals • Award Calculator • Core Budget • Offsite Program Information (Optional) • Outcome Measures • Disqualification Form • Certified Assurances (21st 81 pages 1–4; ASSETs 81 pages 1–3) • Collaborative Signatures • Equitable Access Budget (Optional) • Equitable Access Grant Assurances (Optional) • Family Literacy Budget (Optional) • Family Literacy Grant Assurances (Optional) Note: Application narratives must be attached to the signed forms packet and mailed to the CDE.

  23. Mailing Applications Applicants must mail or hand-deliver an original plus three copies of each signed application packet (including required application narratives) to: After School Division 21st CCLC Grant Application or ASSETs Grant Application California Department of Education 1430 N Street, Suite 3400 Sacramento, CA 95814-5901 Applications must be received by the CDE no later then 5:00 p.m. on November 30, 2012.
