  5. Dental Insurance • Employee pays premium • Provided by Delta Dental of Minnesota • Covered services fall into 4 categories: • Annual maximum per member - $1,000

  6. Vision Insurance • Employee pays Premium • Provided by Superior Vision • Coverage for annual eye exam • Coverage for frames and lenses, or contacts annually • $35 Copay for materials and contact lens fittings

  7. Life Insurance • Term Life • Employee • VCSU pays premium on $3,500 death benefit for the employee • Additional coverage in $5,000 increments up to $200,000 • Dependent/Spouse • Dependent/Spouse – $2,000 or $5,000 option • Spouse – up to ½ of employee coverage Click here to view rates

  8. Long Term Care • Long Term Care (UNUM) • Requires applicants to be underwritten prior to approval • Nursing home care facilities, assisted living facilities, professional home care, or total home care • Coverage is effective the 1st of the month following approval by UNUM

Long-Term Disability • NDPERS (Bands 4000-7000) • After twelve-month eligibility period • Income of 25% of final average salary • Not reduced by other disability payments • The Standard (Bands 0000-3000) • After six-month eligibility period • Income 60-70% of monthly wage base • Reduced by other disability payments

Flexible Spending Program & Premium Conversion • Flexible Spending • Allows employees to pay for qualifying medical expenses and/or day care expenses with tax-deferred dollars • Enrollment is on a calendar year basis • Premium Conversion • Allows employees to deduct premiums on a pre-tax basis • Yearly elections cannot be changed until the new plan year unless there is a qualifying event (birth, death, marriage, etc.) This program is managed by Aflac. Employee Assistance Program • The Village Family Services (Fargo) • VCSU pays premium • Allows 6, 100% confidential, individual sessions per household member • For issues involving relationships, the workplace, emotional health, and service for financial and legal counseling • Contact The Village at 1-800-627-8220

Personal Leave • Sick Leave • Employees accrue 12 days/year at a rate of 1 day/month (prorated to FTE) • Employees may use up to 40 hours sick leave annually to assist ill immediate family • Annual Leave • Eligible employees accrue annual leave from date of hire. Accrual is based on years of service: • May carry over up to 240 hours annually. Must use 40 hours/year after first year.

Perks • Tuition Waiver • For Employee: • Tuition waived for up to three classes per calendar year within NDUS • Release time to attend one class per semester must be worked out with supervisor • For Spouse/Dependent • VCSU provides a 50% tuition waiver for children and spouses of employees attending VCSU • Miscellaneous • Athletic Events • VCSU employees and one guest will be admitted to athletic events by presenting their university ID card. • Graichen Gym Fitness Room • The membership fee for an employee is $70 for the entire 2011-12 season • Viking-I and Cafeteria Meals • Pay for meals in advance and store money on your ID Card to realize savings and no tax on food. • Viking Grounds • Coffee Barista serving Starbucks Coffee in made to order beverages.

Any subject can be discussed, any type of discomfort, or just simply a relaxing talk. Everything is kept confidential. • Fine & Performing Arts • Art gallery exhibits, musical performances, Theatrical performances • Council of State Employees (COSE) • The Council of State Employees prepares numerous discounts for all North Dakota state employees, which can be viewed on their website. • Blue Cross & Blue Shield Wellness Discount • BCBSND members and his or her spouse can earn up to a $20 credit monthly for visiting a participating health club • Blue Cross & Blue Shield My HealthCenter • My HealthCenter is a program designed to help you assess your health, create a plan to improve, and take action. Participation in the program earns points that can be redeemed for gifts and gift cards.
