Changes to Heparin Infusion Documentation: • Name Change to “Heparin (IV Med Drip)” • In addition to Units/kg/hr and Unit/hr. options, drop down is now a multi-select with options for Units/X, Hold, and Stopped • If Heparin is held, document at the time the hold started (or if late charting, back-time to that time). Use if held per order parameter or due to lack of IV Access. • If Heparin infusion is discontinued by order, document exact time the infusion was stopped (or if late charting, back-time to that time) • Any time a Heparin infusion rate is titrated, document new rate at the time the rate change is made (or if late charting, back-time to that time)
