  1. The California State Loan Repayment Program (SLRP) Healthcare Workforce Development Division (HWDD) California Office of Statewide Health Planning and Development (OSHPD)

Purpose, Funding Source, and Administration • SLRP assists with the repayment of educational loans for healthcare professionals who commit to serving in federally designated Health Professional Shortage Areas (HPSA). • The program is federally funded through a grant from the Bureau of Healthcare Workforce (BHW) through the National Health Service Corps (NHSC). • SLRP is administered at the state level by OSHPD.

  3. SLRP Clinician Requirements To apply and be considered for a SLRP award, a clinician must: Be a US citizen (US born or naturalized) Possess a current, unrestricted license to practice in California Be free of judgments arising from federal debt (e.g., no liens) Have no other existing commitment or service obligation to any other entity Not be in breach of a health professional service obligation to federal, state, or local governments Be current on all child support payments Be currently employed or have accepted employment at a SLRP approved practice site Commit to a two-year full-time or four-year half-time service agreement

Eligible Clinician Disciplines Medical Doctors and Doctors of Osteopathic Medicine (MD & DO) Family Medicine, OB/GYN, Geriatrics, General Pediatrics, Psychiatry, General Internal Medicine Physician Assistant (PA) Nurse Practitioner (NP) Pharmacist (PharmD) Certified Nurse Midwife (CNM) Psychiatric Nurse Specialist (PNS) Health Service Psychologist (HSP) Licensed Clinical Social Worker (LCSW) Licensed Professional Counselor (LPC) Licensed Marriage and Family Therapist (LMFT) Doctors of Dental Surgery (DDS) and Doctors of Dental Medicine (DMD) Registered Dental Hygienists (RDH)

SLRP Award Amounts Full-time Awards Half-time Awards • 2-Year Commitment • Total of up to $50,000 • 1st Year (Optional Extension) • Total of up to $20,000 • 2nd Year (Optional Extension) • Total of up to $20,000 • 3rd Year (Optional Extension) • Total of up to $10,000 • 4th Year (Optional Extension) • Total of up to $10,000 • 4-Year Commitment • Total of up to $50,000 • 1st Year (Optional Extension) • Total of up to $10,000 • 2nd Year (Optional Extension) • Total of up to $10,000 • 3rd Year (Optional Extension) • Total of up to $5,000 • 4th Year (Optional Extension) • Total of up to $5,000 Note:Award amounts include funds awarded by the state plus those matched by the site.

SLRP Award Selection Evaluation and Scoring Criteria Training and/or work experience in a HPSA Current or prospective employment in a HPSA Fluency in language(s) other than English Identified as having a disadvantaged background AwardDistribution • Divided between rural, urban, and frontier • Equal distribution between Northern, Central, and Southern California • Equal distribution of clinician disciplines Note: Refer to the Fiscal Year 2019-20 SLRP Grant Guide for more information on the Scoring Criteria

SLRP Site Requirements • Public or Private not-for-profit entity • Located within a California HPSA • Commit to a minimum two year employment contracts with selected providers • Provide outpatient care (primary medical, mental, and dental health) • Match the SLRP award with non-federal funds • Pay clinicians’ prevailing salaries and agree not to reduce salaries due to SLRP support • Provide services on a free or reduced fee • schedule basis to individuals at or below • 200% of the federal poverty level

SLRP Practice Site Profile • To confirm a site’s eligibility for SLRP, a site administrator must: • Contact SLRP directly to request a site administrator user account: SLRP@oshpd.ca.gov. • Log into the funding eApp at https://eapp.oshpd.ca.gov/funding/. • Navigate to your user profile and click Practice Sites/Entity. • Sites administrators should review their practice site profiles ahead of each application cycle.

PracticeSites • Pursuant to Chapter 454 of 2016 (AB 2048), effective January 1, 2017, the SLRP approved practice sites list includes all Federally Qualified Health Centers (FQHCs) • All prospective practice sites (including FQHCs) must create a Practice Site Profile for each clinic in the online eApp system • A Practice Site Profile allows SLRP to verify: • The site is willing to match the SLRP award funds • The Point of Contact responsible for reviewing and signing SLRP award documents

CMSP LRP • CMSP LRP assists with the repayment of qualified educational loans for primary care healthcare professionals who commit to serving at an approved site located in the 35 CMSP counties • The program is funded through a grant from the CMSP Governing Board • CMPS LRP is administered by the California Office of Statewide Health Planning and Development (OSHPD)

CMSP LRP Provider Requirements To apply and be considered for a CMSP LRP award, a clinician must: • Possess a current, unrestricted license to practice in California • Have no other existing commitment or service obligation to any other entity • Not be in breach of a health professional service obligation to federal, state, or local governments • Be current on all child support payments • Be currently employed or have accepted employment at an approved CMSP site • Commit to providing a two-year full-time or two-year half-time service obligation

  15. CMSP LRP Site Requirements • Be located in one of the 35 participating CMSP counties • Have a fully executed contract with the CMSP Governing Board • Be a CMSP approved outpatient facility

Provider Application Process SLRP - Annual Application Filing Period: August 1 – October 1 (subject to change) – eApp CMSP LRP - Annual Application Filing Period: January 2 – March 2 (subject to change) – eApp Providers will be contacted by a program representative if they are chosen to receive an award
