  3. Nutrition Requirements for Fluid Milk • USDA Memo, SP 29-2011. • Persons over two years of age should consume fat-free or low-fat fluid milk. • Fat-free and or Low-fat (1%) milk (unflavored or flavored)

  9. CNPweb Monthly Claim • Monthly reimbursement Claims should be submitted through the CNPweb by the 10th day of the month following the claim period. • Original claims must be submitted within 60 calendar days following the claim month.

Civil Rights Requirements • Complete on-line Civil Rights Training • One person in the organization needs to complete the online quiz • This person is responsible for training the rest of the staff about Civil Rights requirements • Make sure to document the names of the staff that have been trained • Civil Rights training must be conducted annually - http://www.doe.in.gov/food

You may also write a letter containing all of the information requested in the form. Send your completed complaint form or letter to us by mail at U.S. Department of Agriculture, Director, Office of Adjudication, 1400 Independence Avenue, S.W., Washington, D.C. 20250-9410, by fax (202) 690-7442 or email at program.intake@usda.gov. CNPweb Renewal • In May/June, the Sponsor and Site Information Sheets needs to be updated in the CNPweb for next program year. • Must complete the new program year in the CNPweb before July reimbursement claim can be submitted through the CNPweb.   16. Documentations to Retain • Retain all documentation for 3 years, plus the current year • Milk worksheet (if applicable) • Milk invoices/receipts • Quotes • Camp brochure and/or application • On-line Civil Rights Training documentation • Civil Rights training documentation for camp staff
