  3. Executive Level • Responsible for planning strategy • Determining nursing goals • Allocation of resources (human, financial, material) • Leadership in setting goals for the effective and efficient care of patients • Evaluating the quality of nursing services provided

  4. Manager Level • Coordinates nursing activities • Participates in resource management decisions • Serve as the link between the nursing service staff both inter and intra departmentally in the organization • Implements the strategic plan of the organization • Participates in policy formation • Development of staffing and scheduling • Evaluation of performance standard

  5. Software • Both level are involved in the collection and analysis of large quantities of data. • Primary intent of nursing management systems is the provision of information to make decision about the effective and efficient allocation of resources for the highest quality of patient care.

Hours Per Patient Day • Based on Calculation • Number of hours of nursing care provided in 24hours • Number of patient in a 1day • Compared to Standard • Example • 185 hours of nursing (total of all hours of care in 24 hours) • 23 patient days (Census of 23 patients for 24 hours) • Calculation = 185/23 = 8.04. Med/Surg Standard 8.07. Department is within range.

  9. Hours Per Patient Day • Advantages • Calculations simple process of addition and division • Data usually readily available • Disadvantage • Does not account for patient severity of illness; a patient is a patient. • Does not account for unit activity (admissions, transfers, discharges)

Acuity System • Types of systems based on • Self care Deficits – Activities of ADL’s, treatments, medications, & Patient Teaching • Time Based Nursing Activities – Amount of time required to carry out nursing activities such as nursing assessments and physical care. • Patient Care & Nursing Care Requirements – based on needs of patients for nursing care rather than task itself.

Acuity System • Advantage • Calculations can be performed more rapidly than manual system and provide concise reports for managers to review. • Disadvantage • Maintenance of the categorization system and underlying hours of care • Systems need auditing to maintain the reliability and validity of the system

Personnel Management • Focus on Nursing Personnel staffing & employment data • Profiles include general demographic data (Employment history, Educational history, License, absentee rate, Benefit accruals, Evaluations) • Work status data such as daily work schedules and time off requests. • Automated Staff scheduling useful component. Need identification of levels of expertise, nursing workload, patient acuity and scheduling priorities

Personnel Management • Advantages • Less time spent on scheduling • Unbiased assignments • Advanced notice of staff shortages • Availability of payroll data • Disadvantages • Need to develop and maintain rules regarding modification of data • Changes need to be carefully regulated to assure proper scheduling and maintenance of staffing levels.

  14. Fiscal Management • Periodically produced reports allowing the monitoring of budgets, identification of budget variances (actual versus planned expenses) and assistance with planning of future budgets. • Inventory control helps to monitor supplies, process orders, and assist in proper dispensing of supplies.

Fiscal Management • Advantages • Speed with which data can be retrieved, compiled, summarized and presented • Most packages built on Excel functionality • Ability to design reports for individual needs • Disadvantage • Reports can be unusable if too complex to interpret by manager • Report can be unusable if not provided in a timely manner to assist with decision making
