  2. About Form 48 • Purpose is to ensure all cadets meet academic and program requirements in a timely manner. • Cadets tentatively create their degree plan with all courses (including summer) to be taken from entering program to commission. Goal is a 4 year plan or as close to it as possible.

  3. Process for Form 48 • Meet with AS (Aerospace Studies) Instructor to discuss academic and military plans. • Review general education, major, and ROTC requirements through either this PowerPoint, the University catalog, your school/college Advisor, or other resources. • Draft plan. • Meet with major advisor to review draft. • Email draft to ROTC Advisor, Rayna Isaki, at risaki@hawaii.edu for review and verification. Email from your UH account.

  4. Form 48 Requirements • Due mid semester. • Scholarships cadets have Form 48 re-certified by ROTC Academic Advisor every semester while non-scholarship cadets meet only in Fall. • A new Form 48 is required for changes in major, institution, or date of graduation. • Must be full-time (min. 12 credits) every semester; No minimum credit requirement for final semester. • Maximum number of credits per semester is 19. • Online courses limited to 3 per semester and 5 per academic year. • Second majors, minors, and certificates can’t be reflected on Form 48.

  5. Getting Started on Form 48 • Download a blank form by returning to the instructions on the MAC website and viewing #2. • The next slide is what the form looks like…

Complete the First Block: Cadet Information • Enter your name and student ID number. • Enter your institution. • Enter your academic degree & major (e.g. BA Psychology, BS Biology, BBA Business-Marketing). -Include your area of major concentration if applicable (e.g. BBA Business-Finance) • Enter year/month degree will be completed.

  7. Second Block: Review and Certifications • Leave both initial and commissioning review and certifications blank. • Initial review and certification will be signed after meeting with ROTC Advisor. • Commissioning review and certification will be signed just prior to graduation.

  8. Remaining Blocks: All Courses Needed to Graduate Include courses for the following requirements: • General education (gen ed): -Foundation -Diversification • Graduation: -Focus -Hawaiian/Second Language • Major courses in sequential order. • AS courses in sequential order.

General Education Foundations Requirements (designed for the first year of college) Written Communication (FW) 3 credits Symbolic Reasoning (FS) 3 credits Global and Multicultural Perspectives (FG) 6 credits Diversification Requirements (may be taken any time before graduation) Arts/Humanities/Literature (DA/DH/DL) 6 credits Biological Science (DB) 3 credits Physical Science (DP) 3 credits Laboratory Science (DY) 1 credit Social Science (DS) 6 credits

  10. Written Communication (FW) 1 course (3 credits). Must be completed within your first year (2 semesters)! English 100: Composition I (Most students) English 100A: Composition I (Honors) ELI 100: Expository Writing: A Guided Approach (for non-native speakers of English) Transfer students may already have this fulfilled

Symbolic Reasoning (FS) 1 course (3 credits) • Level of math required will depend on major No placement exam required for registration: • ICS 141, Math 100, Math 100A, Nrem 203, Phil 110, Phil 110A, Phil 111, Phil 111A Placement exam required for registration: • Math 140, Math 203, Math 215, Math 241, Math 251A Pre-requisite courses needed to register: • Bus 250, ICS 241, Math 112 To sign up for the Math placement exam, go to www.math.hawaii.edu

Global and Multicultural Perspectives (FG) 2 courses (6 credits) *Must be taken from different groups Group A Group B Group C Anth 151, 151A Amst 150 Bot 105 Art 175 Anth 152, 152A Geog 151, 151A Hist 151 Art 176 LLL 150 Hist 161A Geog 102 Mus 107 WS 175 Hist 152 Rel 150 Hist 155 Hist 162A WS 176 *Any course followed by an “A” designates an Honors course and are reserved for students in the Honors Program only.

Diversification Courses • Arts (DA), Humanities (DH), and Literatures (DL) 2 courses (6 credits) Must come from different groups • Natural Sciences: 7 credits total Biological (DB) 3 credits Physical (DP) 3 credits Laboratory (DY) 1 credit • Social Sciences (DS) 2 courses (6 credits) Must come from different departments For a list of course options for these requirements: http://www.catalog.hawaii.edu/corerequirements/diversification.htm

  14. Graduation Requirements • Focus Hawaiian, Asian, Pacific Issues (H or HAP) 1 course Contemporary Ethical Issues (E or ETH) 1 course (Only offered at upper division level) Oral Communication (O or OC) 1 course (Only offered at upper division level) Writing Intensive (W or WI) 5 courses (2 at the upper division level) • Hawaiian/Second Language Competence at the 202 (or equivalent) level

  15. Focus Requirements • Upper division is any course at the 300 level or above (e.g. ANTH 300, PSY 449, ENG 331). You will likely take these in your junior/senior years and/or within your major. • Focus courses can double count with other requirements, especially major. • May be taken in various departments. -Don’t assume that an H focus must be Hawaiian Studies or O focus must be Speech.

Focus Requirements Continued • Focus courses vary from semester to semester and will be noted only in the Schedule of Classes, not in the Catalog. • Transfer students may have a prorated number of focus requirements. Check http://www.catalog.hawaii.edu/corerequirements/grad-requirements.htm if you have transfer credits. • Spread out W courses over different semesters. It’s recommended not to take more than 2 W courses in the same semester.

Hawaiian/Second Language • Must be completed up to the intermediate (202) level in a single language. -Course sequence is usually 101, 102, 201, and 202. • Placement exam required if you have experience in the language (e.g. studied it in high school, language spoken in house-hold). -Possible back credits. • Assessment required if fluent. -Possible waiver or back credits. • All HSSP (High School Scholarship) winners must take at least 12 credits of a second language regardless of major.

Hawaiian/Second Language Continued The Hawaiian/Second Language requirement has been modified or waived for students in the following schools and colleges: • Shidler College of Business • College of Education • College of Engineering • School of Medicine • School of Nursing and Dental Hygiene • School of Ocean and Earth Science and Technology • School of Social Work • College of Tropical Agriculture and Human Resources For more information, go to: www.hawaii.edu/gened/hsl.htm

Check Your Gen Ed Progress • If you have transfer OR Advanced Placement (AP) credits, check your remaining general education requirements by going to STAR degree check (online academic progress check). • Access STAR by going to www.star.hawaii.edu. • In top left tab, click “academic journey.” • The middle column displays your gen ed progress. This doesn’t display major progress. Check with major Advisor for major progress. • On the right of the gen ed requirement will be either complete or incomplete. You must input on your Form 48 courses for requirements that say incomplete.

How to fill in semester blocks: • Semester= Fall, Spring, or Summer. • Year= Semester year. • Course alpha/#= Course abbreviation and number (eg: ENG 100, HIST 151, MATH 100, etc) • Course title= E.g. “Composition 1” for ENG 100 “Survey of Psychology” for PSY 100. -To check the course alpha/number and title go to http://www.catalog.hawaii.edu/courses/description-index.htm or check the Schedule of Classes. • Identify courses for focus requirements by putting the focus abbreviation in the course title line: E.g. HWST 107 Hawaii: Center of the Pacific (H) THEA 101 Intro to World Drama & Theater (W) -Focus courses are identified in the Schedule of Classes only, not the catalog.

Semester Blocks Continued • Online= Enter Y or N if the course is online or not. • Credits attempted= Number of credits course is worth. -To check the number of credits, go to http://www.catalog.hawaii.edu/courses/description-index.htm or check the Schedule of Classes. • Credits earned= Leave blank for current and future semesters. Fill in only for past semesters. • Grade= Leave blank for current and future semesters. Fill in only for past semesters.

  25. Semester Blocks Continued • TGPA= Term Grade Point Average. Leave blank for current and future semesters. Fill in only for past semesters. • CGPA= Cumulative Grade Point Average. Leave blank for current and future semesters. Fill in only for past semesters. • If more lines are needed for courses than allotted in a semester, combine courses that are a lecture and lab to one line and add credits together. E.g: Biol 171 (3 credits) Biol 171L (1 credit)  Biol 171/L (4 credits) • Make sure the total number of credits for the term is within the minimum (12) and maximum (19).

  29. What’s next? After completing your Form 48 with all necessary requirements: 1) Check your draft with your major Advisor. 2) Email draft to ROTC Advisor, Rayna Isaki, at risaki@hawaii.edu for review. Email from UH account. 3) Revisions (if any) will be returned. Make revisions and resend revised Form 48. 4) Meet with ROTC Advisor for signature. 5) Meet with AFROTC instructor for signature and submission.
