  1. Longitudinal Care Work Group(LCWG) Proposal to Re-Scope and Re-Name the LTPAC WG

  2. Vision • Promote Longitudinal Care Management built around the needs and experience of the individual • Include all relevant sites and providers • Develop four critical components to achieve this goal, each provided by a sub-group coordinated by the WG leadership

  3. Purpose Vision: Support and advance person-centric interoperable health information exchange Focus on three broad use cases: Longitudinal Care Plan Patient Assessment Summary Documents Patient Transfer Documents Goals: Contribute to S&I Framework CIM and HL7 Support MU Stage 3 Identify standards, implementation specifications, and certification criteria that could be adopted by the Secretary of HHS

Work Partitioned to Four Sub-Groups • Longitudinal Care Plan (TC, RL, WR) • Identify components of plan • Propose required functionality • Consideration access, editing, “control of plan” • Exchange of Patient Assessment Summary Document- (JY, JH, SM) • Define domains, tools, definitions • Data exchange at Transitions of Care (LG,TO) • Data coordination across Sub-groups (JH, WR, SM,LG,TO) Participation is not limited to those on this slide, nor is anyone limited to just one sub-group

Cross Sub-group Data Team Data access- coordinate across subgroups 1-3 to: • Align data standards applied to MDSv3 and OASIS-C with standards in the CIM, • Provide support to subgroups 1-3 for the re-use of: standardized assessment data elements [and document exchange standards] • Support the Integration of the deliverables of the LCWG into MU Program, ONC goals, and as possible, CARE Tool (JH, WR, SM) • Engage the Health IT Policy and Standards Committees on the needs for and gaps be filled by the LCWG • Ensure Subgroups collaborate with HL7 as needed (e.g., Care Plan workgroup) • Reach out to CMS/Lantana to participate in LTWG

Role of Sub-Group Leaders • Recruit committed members from broad range of stakeholder groups • Coordinate discussion of committed members • Coordinate with existing efforts, tools and standards in their domain • Ensure consistency and alignment of work across sub teams as part of WG Leadership Group weekly call • Produce summary of work products to be included in the final output of the LCWG

Role of Co-Chairs • Support the work of the sub-groups • With the Sub-group leads and other committed members constitute the LCWG Leadership Group which will: • Reconcile the work of the sub groups • Assure that the work group outputs are on target • Assure that the output of the LCWG is consistent with S&I, MU, CMS and ONC goals • Prepare the final product of the LCWG • Identify opportunities to advance the objectives and deliverables of the LCWG

Output From Each Subgroup Describe pertinent use case(s) Identify relevant work/efforts/tools/standards Define subsets of data elements pertinent to each use case Identify and fill gaps in current CIM/CDA content (changes and additions) to satisfy all of subgroup’s use case(s) Map these data elements to domain’s existing tools/documents (AKA Rosetta Stone) Create CDA Implementation Guide(s) Propose pilot implementations

S&I ToC and LCWG Timelines TOC Initiative to complete roadmap and contribute to CIM due Feb. for HIMSS conference LCWG Leadership Group: will seek to have LCWG activities included at HIMSS Town Hall Each LCWG Sub-group has its own time line LCWG Leadership Group will: Determine publishing date for MU Stage 3 as critical end point for work product Be consistent with any proposed time-line generated from the Convening Conference 10/14/11

ONC Support Meredith Lewis/Eric Pupo/Dev Dasgupta/Ashley Swain Deloitte consultants to support ToC S&I Support CIM (Clinical Information Model) Produce S&I process wide information model Each work group will contribute to final doc Preliminary process outlined in Longitudinal Care/LTPAC Road Map Help with logistics (Wiki, Webex), coordinate across S&I work groups

  11. Committed Members Facilitate engagement Simplify application Provide sample statement Committed Member definition Anyone willing to contribute to the effort Review and comment in weekly calls Highly Committed Members Participate on Leadership Group Sub-group work assignment
