This case study by Stephanie Sadek at Baker College of Auburn Hills reviews a 38-year-old female patient undergoing a carotid duplex exam. The patient presented with symptoms including headaches and dizziness, linked to fibromuscular dysplasia (FMD), characterized by abnormal arterial growth. The report indicated normal parameters for certain arteries but highlighted accelerated flow velocities in the distal internal carotid arteries. Treatment options include surgical intervention and management of hypertension, with attention to potential complications such as stroke and chronic kidney failure. ###

Fibromuscular Dysplasia • Definition : Condition when arteries have abnormal cluster of cells growing on wall. • Bead-like appearance. • Most often affects renal arteries and carotid arteries. • Symptoms include : dizziness, blurred or temporary loss of vision, neck pain, chronic headaches, facial weakness or numbness. Mayo Clinic
CTA of head and neck report : The distal cervical internal carotid arteries bilaterally have a irregular contour, resembling a “string of beads.”
Preliminary Report • R. Proximal CCA, distal CCA, bulb, proximal ICA, and ECA appear within normal limits. R. distal ICA displays accelerated flow velocity of 244 cm/sec PSV; 107 cm/sec EDV. • L. Proximal CCA, distal CCA, bulb, proximal ICA, and ECA appear within normal limits. L. distal ICA displays accelerated flow velocity of 166 cm/sec PSV; 83 cm/sec EDV. • No plaque visualized. Both vertebral’s have antegrade flow.
Treatment • Surgery to open narrowed portion of the arteries. • Medications for hypertension. Mayo Clinic
Conclusion • Complications of FMD include : • High blood pressure • Chronic kidney failure • Dissected artery • Aneurysms • Stroke Mayo Clinic
