  1. Transforming people from homelessness to self-reliance in order to improve their lives and the Midlands community.

Floyd • Worked over 20 years as a security guard in the Midlands. • Within 2 months of losing his job, he was diagnosed with stomach cancer. • Without health insurance and limited savings, bills begin to pile. • Became homeless • He arrived at Transitions in March 2012. • Became an inspirational role-model to both his fellow residents and the staff. • During his stay at Transitions he underwent Chemotherapy. • In remission now • In March 2013, Floyd graduated from Transitions’ program. • With the help of his Transitions’ case manager, he obtained disability and permanent housing. • In May 2013, Transitions’ Board of Directors invited Floyd to join as a full board member, to represent current and former clients. 2

Tammarria • Suffered from substance abuse (crack cocaine) for many years. • She arrived at Transitions in 2011; she was a client for 18 months. • Transitions offered stability, hope, encouragement, and resources needed to change her life. • Transitions’ case manager Mrs. English changed her life; she gave Tammarria a helping hand versus a handout. • Received help from LRADAC, and she has been clean for 2 years and 4 months. • Regained custody of her children! • During her stay at Transitions, she obtained two jobs– as a custodian and a supervisor/manager at IHOP. • Still attends NA and AA meetings. • Has been happily married for 1 year. • Obtained permanent housing! • After IHOP closed, she obtained employment at Transitions in Day Center. • Future Plans--- Hopes to become a Social Worker specializing in Drug and Addictions counseling. She wants to help women like her in conquering addictions! 3

ACCOMPLISHMENTS (June 15, 2011- October 29, 2013) • 450 clients moved to permanent housing • 1302 clients moved from homelessness to a more positive living situation • Engaged over 2,587 unique clients last year • $16.00 per night to provide services to someone who is currently homeless • 154,537 nutritious meals served last year (Over 250K since opening) 4
