  4. Adding an Entity Add all required information New Entity will provide Day Care Services Find the Standard Industry Classification Codes

  5. Add Location and Human Services Add all required information Entity has been added successfully Public comment Business Function must be chosen

Human Service Providers • Identifying a Contact as a Human Service Provider or Training Provider will allow the Provider to maintain their own information • By checking either Human Service Provider or ITA Training Provider the application will generate a Username • LWA staff with the proper Role can Reset the password • The Provider could then be given the URL and Password which will allow them to enter the system and maintain Location, Contact, or Human Service Provider Information
