  2. Experience in Practice Redesign • Practice redesign in our practices • CCNC regional network • Community Care of North Carolina, NC AHEC Improving Performance in Practice • I3 and I3PCMH collaborative of 25 residencies in FM, IM, Peds • UNC collaboration with BCBS

  3. Priorities at Practice Level • Advanced access first… • Why not improve continuity? • Multiple comorbidities /embedded care management • Money not in readmissions but in total admissions and ED

  4. Priorities at the Regional Level • CCNC: regional financial, informatics, care management • Interventions med reconciliation, transitions, ED visit prevention, etc… • Current target:  25% hospitalizations, and  50% ED visits, MD referrals and CT/MRI

  5. Raising a Barn Statewide… • Regional nodes • Community based infrastructure for case management and ongoing practice redesign; • Multipayer informatics, organized by practice • How to fit integrated systems/ACOs across states?

  6. Priorities for Now… • Sustainabilitythrough prevention of hospitalizations, ED visits, medical consultations • Incentivesand the right kind of infrastructure… • Aligning individual responsibility with team contributions
