  3. Officer Eligibility • Undergraduate Students • 9 credit hours • 2.5 cumulative GPA • Graduate Students • 6 credit hours • 3.0 cumulative GPA • Good academic standing • Free of conduct probation

  4. Membership Forms • Liability Waiver • Emergency Contact • Player Conduct • Copies available in the SCO • Due on first day of practice OR 9/25, which ever comes first

  5. Team Travel • Travel Forms • Due one week prior to trip • Late Forms: reimbursements are not permitted • Missing Forms: purchases will be canceled (hotel, car rental, etc.) and reimbursements are not permitted • Emergency Contacts • Take with you on all trips

  6. Accident & Incident Reports • What do you need to report? • Location: on campus, off campus or during trips • Event:accident/injury or incident between students, visiting teams, club members, or spectators - at any FGCU related event • Call or email - we want to hear it from you first!

  7. Risk Management • Certifications can be obtained through Campus Rec, copies of cards must be submitted to the SCO • Risk Level Two • 2 safety officers • Risk Level Three • 2 safety officers • and coach certification or1 additional safety officers • Risk Level Four • 2 safety officers • and coach certification or 2 additional safety officers

  8. Risk Management • First Aid Kits • Available for checkout from office • First Responders • For all home events • Athletic Trainers or EMTs • For higher risk home events

  9. Concussion Policy • Baseline ImPACT Testing • M Lacrosse, Rugby, Soccer, Hockey, Wrestling • Post-Injury Testing for All Clubs • Managing an Injury • Remove the athlete from play • Keep the athlete out the play until a *qualified physician* gives written clearance

  10. Hazing • Any action or situation which recklessly or intentionally endangers the safety of a student as a condition of membership • National Hazing Prevention Week • September 23-27 • SCORE points offered for attending Movie Night and Keynote Speaker

  11. Reminders • Eagle Head logo can be used by Sport Clubs • All items must be printed by licensed vendor • EagleLink – update your officer list! • Next SCC Meeting on 9/28 in SU 247 • CPR/FA certification proof due for at least 2 active club members
