  2. Keep in mind… • Only 1 initiative evaluated per grant. • Initiative year may be different than grant year. • Program Measure 2 was only evaluated for initiatives in their 3rd or 4th year. • Not many grantees report on Program Measure 4.

  4. Considerations for the current APR • Program Measure 1: • Use bullets in your descriptions that align with bullets in the first column. • Provide your initiative name & yr in the worksheet. Julie Morrison, Ohio and Michigan’s SPDG evaluator

  5. Program Measure 2: • Just use one fidelity assessment as your program measure. • You can include your other fidelity measures as Project Measures • Name your fidelity measure and describe how it’s a fidelity measure. • Attach your fidelity measures to your APR

  6. Program Measure 3: • Provide the actual dollars spent in the equation. • Ongoing TA definition
