4. How to collect race/ethnicity Ask these two questions: • Do you identify yourself (or the child) as Latino or Hispanic? • Do you identify yourself (or the child) as: • American Indian or Alaska Native • Asian • Black or African American • Native Hawaiian or Other Pacific Islander • White

5. Handwritten COE • Enter all the selections • COE  in Race/Ethnicity column enter the following letters: • AM = American Indian or Alaska Native • AS = Asian • BL = Black or African American • HI = Hispanic or Latino • WH = White • PI = Native Hawaiian or Other Pacific Islander
