SAH HIV AND AIDS COORDINATING TEAM COMMITTEE LABORATORY FLOW CHART VOLUNTARY CONSENT FORM SECURED BLOOD EXTRACTION Extract blood sample observing standard precaution principle Test will perform by HIV Proficient Medical Technologist Using rapid test If REACTIVE, repeat the test If NON REACTIVE (Written Result will be Released to HACT for post test counseling) Record the result Forward the result to the HACT for post test counseling. (Test should be repeated after 3 months and another 3 months before he/she be cleared for HIV.) Do a referral form address it to SACCL together with the consent form for Confirmatory Send specimen to SACCL SACCL result NOTED BY GRACE H. PADILLA,MD HACT TEAMLEADER
