ILEC ICB Process Overview • The Contract Administration team is responsible for the post-sale management of ILEC ICB contracts Functions include: • Facilitate implementation • Assure regulatory compliance • Coordinate bill code assignments • Notify Customer Service to move forward with contract implementation • Monitor contracts annually and notify Sales/VPS of shortfall debits due • Manage contract renewals/expirations with Sales monthly, and disposition as appropriate • Key players in ILEC ICB post-sales contract processing are Sales, Customer Service and Contract Administration

Regulatory Guidelines For intrastate services under the jurisdiction of WV Public Service Commission: • WV ICB - Tariffed service(s) can be provided under an individual case basis (ICB) contract when necessary to respond to competition. Per WV PSC orders in Case No. 06-1935-T-P, Verizon is no longer required to file summary reports; however, ICB contracts must be made available if requested. • WV MSA - Equipment for use with universal E911 services has been rate deregulated in WV with PSC Order No. 04-0292-T-PC, effective 8/10/04. Contracts must be made available if requested. • WV Special Equipment and Service Arrangement Contract - A special equipment and service arrangement contract is permitted for unique equipment and service arrangements not offered in a general tariff. Rates must be sufficient to provide for recovery of all relevant costs. For interstate services under jurisdiction of the FCC: • On March 19, 2006, Verizon received forbearance from the FCC on its Optical Services (i.e.: ATM, Frame Relay, Ethernet, TLS, DWR, SONET) which essentially deregulated this group of services. • Most other FCC services are sold at tariff; if they are sold via ICB they must be filed with the Commission.

Billing Codes • For WV • Local ILEC ICB products are billed in a system which automatically reverts to tariff rates at the end of the contract term. • IntraLATA usage services automatically revert to tariff rates at the end of the contract term. • Only Virtual Flat Rate automatically bills threshold overages • Corporate Rewards is not an ICB, it is in the tariff • Interstate ICB products are billed in CABS and do not automatically revert to tariff/retail rates at the end of the contract term. • Customer commitment monitoring is manually performed on an annual basis and any associated shortfalls are billed manually.

  5. ILEC ICB Post-Sale Management - Tools • ICB contracts are scanned into the Document Management System and flow through a batch interface to Oneview. Oneview has workflow functionality that assigns and tracks progress through the post-sale process of regulatory filings, tabling code activities, customer service order completion and NCA contract activation. • For WV, Verizon will provide a data extract from Oneview including the following information • Customer Name, Contract Execution Date, Miscellaneous Tracking, Notes, Service Types, State (WV), Address, City, State, Zip code, Country, Contract Profile ID, Contract Profile Version, Amended/Renewal Profile ID, Amended/Renewal Version, Parent Multi-Jurisdiction (MJ) Profile ID, Parent MJ Profile Version, Amendment Effective Date, Amendment Execution Date, CBSCNE Case number, ICB Number, Contract Profile Status, Contract Start Date, Contract Start Date Reason, Contract Expiration Date, Contract Term, Extension Language, Jurisdiction, Multi-Jurisdiction Indicator, Billing Product Type, Product Code, Product Description, Rate, Quantity, BTN • Contract Statuses include: Active, Activation Pending, Inactive Pending, New Request, Profile Created, PCM/NCA Prepared, Regulatory Filing Complete, PUC/FCC Approved, Billing Code Complete, IOSCs Identified, Tabling Complete, SODS Pending, SODS Complete, SODS Returned, SODS Reworked, Bill Verification Complete, Monitoring Plan Complete, Inactive Amended, On-Hold
