African Regional Goal for Measles Elimination by 2020 • > 90% coverage with the MCV1 nationally (WHO UNICEF estimates) AND > 80% vaccination coverage in every district or equivalent administrative unit in all countries. • >95% coverage with measles vaccines during SIAs in every district. • incidence of < 1 confirmed measles case per 1,000,000 population per year (excluding imported cases). • Achieve the surveillance performance targets:

  4. Regional MCV1 coverage (WHO-UNICEF estimates) and countries with >90% coverage. 2001 – 2011.

  5. Infant cohorts not vaccinated with MCV1. African Region. 2011. 50% 80%

  6. MCV2 introduction in AFR. November 2012 • At least 30 countries are expected to have introduced MCV2 by the end of 2015.

  7. Measles SIAs

  8. Measles SIAs. AFR. 2001 – Nov 2012 • 578 million children reached since 2001. • An average of 40 – 50 million children reached every year. • More than USD 27.5 million raised locally for SIAs in 2011 – 2012. • 9 of 12 (75%) countries raised more than 50% ops costs for SIAs in 2012.

Performance against the 2020 measles elimination targets (as of Dec 2012) • > 90% MCV1 coverage nationally (WHO UNICEF estimates) • 16 countries (end 2011) • >95% coverage during measles SIAs in every district. • 3/12 countries (2012) • incidence < 1 confirmed measles /1,000,000 pop’n /year • 16 countries • NMFRI rate of > 2:100,000 • 25 countries • > 80% districts reporting per year • 25 countries

  14. Main Challenges • Measles outbreaks in the face of: • gaps in immunisation service delivery • Shifting epidemiological susceptibility towards older age groups • Gaps/ delays in local resource mobilisation

Opportunities ahead for measles elimination in AFR • Maintaining the polio infrastructure to continue to benefit measles elimination efforts • IPA/ AAP - advocacy with the national paediatric societies • GAVI support: • for SIAs in selected countries • for wide age group MR SIAs : • for outbreak response • Efforts to reprogram/ realign HSS support to immunisation program outcomes in specific countries
