  3. Introduction • MSP is the only process through which new members are initiated into the fraternity. • Key Roles: • District Representative • District MSP Chairman • State Representative • Chapter Basileus • Code of Conduct / Zero Tolerance For Hazing • If you haze, then you WILL BE EXPELLED!

  4. Introduction • MSP is divided into four (4) general phases • Selection Phase • Application Phase • Education Phase • Initiation Phase

Membership Requirements – General To become a member of the Omega Psi Phi Fraternity, Inc., prospective candidates must: • Possess the qualities of Manhood, Scholarship, Perseverance, and Uplift; • Display qualities of leadership; • Show evidence of school and/or community involvement ; • Request the college/university registrar’s office to send an official transcript bearing the college/university seal to the District Membership Selection Chairman or State Representative;

Membership Requirements – General (con’t) • Participate in an official Informational Session conducted by the District Membership Selection Chairman or State Representative; • Pay all dues, fees, and assessments on time; • Submit a medical examination (taken within the last 90 days); • Pass a records check, formal interview, and written examination; • Submit three letters of recommendation from the community at large;

Membership Requirements – Undergraduates • Must have earned at least 36 semester credit hours (or equivalent) of college credit; • Must have a grade point average of at least 2.5 or higher on a 4.0 scale (or equivalent on another scale); • Must be certified as a registered full-time student working toward a baccalaureate degree; • Must satisfy any and all other qualifications as may be imposed by the college/university;

Membership Requirements – Graduates • Must have earned at least a bachelor’s degree from an approved and accredited four-year college/university; • If candidate graduated from a college/university where there was an active Omega undergraduate chapter, then said candidate must wait at least 4 years after graduation before applying for membership.

  11. MSP Initiation Fees • Money orders or cashier’s checks are the ONLY acceptable forms of payment. • Initiation fees must be submitted along with application. • Money order #1 must be made payable to “OMEGA PSI PHI FRATERNITY, INC.” • Money order #2 must be made payable to “TENTH DISTRICT”

Key Forms / Documents • Form 1 – Notice of Chapter Request • Notifies DR that chapter wishes to conduct an MSP. • Approval of Form 1 only authorizes chapters to select candidates and to compile a Form 105 Package. • Form 1 approval DOES NOT CONSTITUTE authorization to distribute applications, conduct education phase, or conduct an initiation.

Key Forms / Documents • Form 105 Package – Request For MSP • Formal request for an MSP for specific candidates. • Key Form 105 Package documents are: • Form 105 – Request for MSP • Form 1E – List of Approved Candidates • Attachment to Form 105 – Nationally Mandated Programs • Form 37 – Report of Chapter • Form 50 – Chapter Officers • Form 53 – Chapter Roster • Form 9A-1 – Nomination Portfolios (1 per candidate) • Submit Form 105 Package to District MSP Chairman

  14. Key Forms / Documents • Form 10 – Certification Of Men For Initiation • MUST BE INCLUDED IN MSP PACKAGE! CANDIDATES MUST SIGN PAGE 2! • State Representative (or District MSP Chairman) should have prospective candidates sign page 2 at the Informational Session. • If undergraduate candidates are involved, then page 1 must also be stamped/signed by the registrar’s office at the university. • If undergraduates are from more than one university, then include one Form 10 per university. • DO NOT COMPLETE INITIATION DATE FIELDS. Leave blank for District MSP Chairman and DR to complete.

  15. Key Forms / Documents • Form 3 – Fraternity Pin Order Form • MUST BE INCLUDED IN MSP PACKAGE! CANDIDATES MUST SIGN PAGE 2! • State Representative (or District MSP Chairman) should have prospective candidates sign page 2 at the Informational Session. • One Form 3 can be used for all candidates (graduate and undergraduate)

Application For Membership Refer to handout (limited copies available) Application Notes: • Passport Photographs • MUST submit two (2) passport photographs. • Photographs must clearly show candidate wearing a suit jacket, collared dress shirt, and tie. • PRINT candidate’s full name and chapter name on back of passport photographs.

  17. Application For Membership Application Notes (con’t) • Money Orders: • Validate money order amounts • Validate “Pay to the Order of…” name. • Validate that candidate’s name is clearly PRINTED on the front of each money order (use memo section or remitter section).

Application For Membership Application Notes (con’t) • Letters of Recommendation: • Only professionally-written letters of recommendation will be accepted! • Letters of recommendation must be written in standard business format. • All letters of recommendation must be typed! (no handwritten letters of recommendation!) • Failure to adhere to these standards shall be cause for rejecting the candidate.

  19. Application For Membership Application Notes (con’t) • Validate that candidates have signed and/or notarized all necessary documents in the application! • Notary public stamps must appear on pages 7, 8, 11, 19, and 22 of the application.

  20. Key Points / Summary • Only the DR, State Representative, or District MSP Chairman are authorized to conduct the Informational Session and the initiation ceremony! • Candidates must NOT use the old application form! • Education Phase can only begin after the DR has received approval from the IHQ.
