This starts with risk stratification of chronically ill patients. • Individualized care management programs are built for high risk patients in order to prevent or delay hospitalizations. • The primary care performance is measured on the basis of improvement in population state of health (SOH), (measured as PQRS or ACO quality metrics), the cost of care, and patient experience.
