Wisconsin Sound Beginnings 3-Step-Follow-Up Process WSB Central Staff identify infants who referred on their inpatient screen, are greater than 30 days of age & timed-out in WE-TRAC (because they haven’t received outpatient screen by 1 month) Step 1: Medical Home Outreach Central Staff contacts Birth Units & Primary Care Physicians to verify infant’s hearing screen status & contact information Case Resolved Assign case to Follow-Through Parent Guide (& place WIC alert) Step 2: Parent-to-Parent Outreach FT Guide contacts family & if they won’t/can’t access traditional care, sends letter FT Guide contacts family & guides them into traditional care If FT Guide can’t reach family, sends letter Step 3: Regional Outreach FT Guide monitors case, maintains contact with screening organization & family & determines next steps Case transferred to Regional Outreach Specialist If no follow-up imminent, GBYS sends letter Case Resolved ROS contacts family & helps them get into traditional care ROS contacts family & local public health agency & arranges for in-home or community screen Case Resolved or Case Closed after all efforts exhausted RM 2/28/2012
