Behandeling Low-flow Priapism • Doel: Erectie stoppen Voorkomen schade corpora cavernosa Verlichten pijn • Puncteren corpus cavernosum • Alfa-adrenerge agonist injecteren (<12 h) • Epinefrine, fenylefrine • Iedere 5 minuten herhalen • Terbutaline oraal (5 of 10 mg) • Priapisme agv ICI • 36 % succes

  18. Behandeling High-flow Priapism • Vaak delayed onset • Methyleen-blauw • Tegengaan NO release • Arteriografie en embolisatie art pudenda interna • Percutane autologe stolsel embolisatie van sinus cavernosus

  19. Behandeling: Recurrent Priapism (RIIP) • Als ischemisch • Preventie belangrijk • Hormonaal • Niet hormonaal • Chirurgisch

  20. Behandeling RIIP: Hormonaal • GnRH agonist/antagonist • Verlagen circulerend testosterone niveau tot < 200 ng/dL (treshhold nachtelijke erectie) • Bijwerkingen veel: • Geen sexuele erectie, gynaecomastie, trombose, futlossheid, depressie, osteopenie/porose

  21. Behandeling RIIP: Hormonaal • Androgeen receptor antagonist • Flutamide, biclutamide 1dd 50 mg • Pijnlijke gynaecomastie • Ketoconazol • 200 mg 2-3 dd • Reductie circ testosteron 200 ng/dL • Inhibitie cytochroom P450 afhankelijke enzymene bijnier/testis -> inhibitie biosynthese testosteron EN mineralocorticoiden • Dus: + Predinos (1dd 2,5 mg)

  22. Behandeling RIIP: Niet hormonaal • Baclofen • Gamma-aminoboterzuur agonist, gebruikt bij spasticiteit. Intrathecale toediening gaf ED • 20-40 mg ante noctum effectief met sparen sex. erecties

  23. Behandeling RIIP: Niet hormonaal • Digoxine • Inotropicum, cardiale medicatie • Inhibitie Na/K-pomp ATPase • Ook gladde spiercel corpus cavernosum 1dd 0,5 mg

  24. Behandeling RIIP: Niet hormonaal • Neurontin • Anticonvulsief, antinociceptief en anxiolyitisch • ED als bijwerking 4 dd 400 mg bij start 900 mg/dag later

  25. Behandeling RIIP: Niet hormonaal • Zelf-injectie fenylefrine • Bij erecties > 1-2 uur • 100-500 microgram/ml • 1 ml iedere 3-5 minuten • Maximaal 1 uur proberen

  26. Behandeling: Chirurgisch • Shunting • Implantatie penis prothese
