  2. Coding regulation • ผู้ให้รหัสสามารถให้รหัสได้เฉพาะการผ่าตัดหรือหัตถการที่แพทย์บันทึกใน discharge summary • ถ้าเห็นว่าแพทย์บันทึกการผ่าตัดหรือหัตถการไม่ครบ ต้องปรึกษาแพทย์และขอให้บันทึกเพิ่มให้ครบ • ถ้าแพทย์บันทึกผิดหรือบันทึกไม่ ละเอียดย่อมมีโอกาสได้รหัสผิดสูง เพราะผู้ให้รหัสไม่ใช่แพทย์

Terminology • Open operationหมายถึง การทำผ่าตัดโดยการเปิดหน้าท้อง • Closed operationหมายถึง การทำผ่าตัดโดยใช้กล้องส่องภายใน (endoscope) • Endoscopeหมายถึง กล้องส่องภายใน ใช้ในการตรวจและการผ่าตัด ที่มีใช้ในปัจจุบันได้แก่ laparoscope (กล้องส่องช่องท้อง) และ hysteroscope (กล้องส่องโพรงมดลูก) • Other หมายถึง การผ่าตัดที่แพทย์ระบุชื่อเป็นอย่างอื่น นอกเหนือจากการผ่าตัดที่กล่าวถึงก่อนหน้า • Synchronous procedures หมายถึง หัตถการที่ทำในคราวเดียวกัน
