Description of the activities and main tasks: • Project Management: • Meetings of the Steering, Evaluation and Communication Committee in order to plan and examine the envisaged transnational activities: • Elaboration of a transnational Work Plan; • Revision of the previous events and planning of the future ones; • Monitoring of the progresses of the partnership and of the Work Programme One meeting is planned in each partner country

  4. Experts bilateral meetings • Meeting in Denmark: • Visit to private and public organisations able to transfer best practices on rehabilitation, inclusion into the labour market and social integration of disabled people

Transnational networking for thematic exchanges of information, experiences and best practices • Information exchange on partners’ experiences; • Experts, consultants and professionals exchanges; • The activity will be carried out through teleworking; • Web site as digital framework made available by the Polish partner; • Creation of a virtual forum for the discussion, the exchange and the dissemination of information and project results • Creation of a link to the dedicated web site by all partners

Compared analysis of experiences and practices tested on the involved disadvantaged people • Exchanges of national reports on researches and on the outputs of activities of the national projects; • Elaboration of a Best practice Handbook; • Lithuanian partner in charge of the drawing up of the Handbook; • Dissemination and diffusion of the Handbook through the dedicated web site and other tools

Joint development of methodologies, approaches and operational tools in the same Equal thematic field • Establishment of working groups of the different DPs involved in the project: • periodical meetings; • involvement of all partners; • definition and development of innovative models, methodologies and tools for fostering the employment of the target group

Dissemination, communication and diffusion • The transnational communication will be ensured by the Polish partner through the Web site; • Each partner will elaborate dissemination tools (leaflets, brochures, project fiches, etc.); • Workshops for the diffusion and dissemination of the national and transnational activity, of innovative methodologies, ideas and best practices will be organised

Procedure di monitoraggio e valutazione • Attività realizzata dal Comitato di Pilotaggio, Valutazione e Comunicazione; • Elaborazione di report intermedi da presentare agli incontri del CPVC; • Gestione del sistema assicurata dal partner Italiano PUG 005.

Responsabilità di ciascuna PS Ogni P.S. partner si incarica di: • Costituire il Comitato transnazionale di Pilotaggio e Valutazione • Indicare un referente transnazionale che sarà l’interlocutore privilegiato per il segretariato dell’ A.C.T. • Scambiare metodologie e strumenti sperimentati attraverso ogni progetto nazionale • Organizzare l’accoglienza dei partner e le attività previste per i meeting transnazionali • Definire il programma degli incontri transnazionali che si realizzeranno presso la propria sede • Redigere il verbale degli incontri transnazionali svolti • Partecipare agli incontri previsti dal C.P.E.C.

  13. Indicare identità e indirizzi degli esperti e dei consulenti che partecipano agli scambi transnazionali • Comunicare ai partner transnazionali i report delle attività nazionali realizzate • Partecipare attivamente alla Valutazione delle attività transnazionali sotto la responsabilità del partner italiano – PUG-005 • Contribuire alla realizzazione della guida di buone pratiche sulla base delle sue proprie esperienze e sotto il coordinamento del partner della Lituania • Assicurare una buona disseminazione dei risultati a livello locale, nazionale ed europeo

  14. Inoltre: • Il partner polacco mette a disposizione una parte del suo sito web per la creazione di un forum di discussione per i partner. Il sito permetterà anche gli scambi di strumenti e prodotti realizzati.
