Compounding Pharmacies503A vs. 503 B 503A 503B Compounding entity that may register as an outsourcing facility Must compound in compliance with CGMP or under direct supervision of a licensed pharmacist in a registered facility Must also report information to the FDA about compounded drugs such as source of ingredients Must adhere to new quality standards • Compounders not registered as outsourcing facilities • Only exempt from certain aspects of 503B • Still must meet all applicable conditions of the FDCA

  7. Tighten Up!(On Medical Record Documentation) Current Focus of Medical Record Audit: • Documentation of Allergies and Reactions • Completion of Medication Reconciliation, if applicable • Physician order to accompany all administered meds • Use of unapproved abbreviations

  8. Medical Record AuditEarly Findings • Unapproved abbreviations most commonly used: • “cc” is being used instead of “ml” • “qd” being used • Continued use of AM/PM rather than 2400 • Documentation of allergies, but no documentation of sensitivities
