  3. What is the AMGF? The American Medical Group Foundation (AMGF) is the 501 (c) (3) affiliate of the American Medical Group Association and it supports and disseminates research on better ways to deliver health care that is scientifically sound and value-based. AMGF also supports educational programs to enable all stakeholders―providers, payers, policymakers and patients―to translate the evidence of what works best into everyday practice.

  4. What is the Acclaim Award? Honors physician-directed, patient-centered medical groups and other organized systems of care that have embraced continuous learning and innovation in order to improve the quality and value of the patient experience. Mission To honor the nation’s premier healthcare delivery organizations that are high performing and are bringing their organization closer to the ideal medical group and health system by: • Measurably improving the quality and value of care • Improving patient experience and outcomes • Continuously learning and innovating • Improving population health

  5. Acclaim Award History • 1999 – Began as a way of recognizing Medical Groups for outstanding improvement in patient outcomes. Initially co-sponsored by AMGA and Pharmacia & Upjohn. • 2004 – Structured around the six IOM Aims • 2007 – AMGA Board decided the Acclaim Award should be sponsored solely by the association • 2009 – Acclaim Award celebrated 10 years • 2012 – Mission and core values was created • 2013 – Criteria modified around AMGA’s definition of a High Performing Health System (HPHS)

What has changed? • Added expectations of recipients and honorees • Specified the need for examples throughout the application • Changes to the narratives to include a new narrative on innovation • New evaluation criteria and rating • Page count What has stayed the same? • IOM Aims • Application process • Time allotted to complete the application

Expectations of Recipients & Honorees The recipient and honorees will have the opportunity to: • Present award-winning initiative during one of the IQL Conference peer to peer sessions • Publish stories of their award-winning initiatives in AMGA’s Group Practice Journal (GPJ) • Participate in an educational session during Annual Conference (IQL pre-conference) • Receive a marketing packet to assist with promotion • Participate in educational webinars and meetings to further share their best practices

  8. Narrative Changes Section 1 Narrative 1. An overview of your organization today and what it will look like in the future. • What were your most important strategic challenges? Section 3 Narrative -new section 3. Describe how your organization is continuing to learn and innovate? Section 4 Narrative 4. Describe your step-by-step implementation plan. • Who was involved in designing the implementation plan and planning the redesign? What skills/relationships did they bring to the table? • How did you involve patients in the improvement design and implementation? • How did you involve all levels of staff in your improvement design and implementation?

  9. Narrative Changes Section 5 Narrative 5. Describe your process for determining if you met your goals and share your results. • What were your results? Please provide specific examples with data such as: national benchmarks used, averages vs 90th percentile, pilot vs whole group impact. • Describe how population health was improved? What population of patients was impacted? (Numerator/Denominator) • How did you measure patient experience and what were the results? • What did not work? • How could industry leaders learn from your journey?

Review Process Who reviews and selects the Recipient and Honorees? • Expert Advisory and Review Panel selected by AMGF Two-tiered Review Process: 1st Tier • Aminimum of 2-3 panelist is assigned to each applicant and are asked evaluate and score on each criteria as well as provide feedback on the strengths and weaknesses of the application. • Narrow down to the Top 10 applications. 2nd Tier • All panelist to read through all of the Top 10 applications. • Each panelist assigned 1-2 applications to present • 2 hour conference call where each of the Top 10 applications are presented by a panelist • Panelist email their votes into AMGF, each is allowed to select 1 Grand Prize (Acclaim Award Recipient) and up to 3 Honorees.

Checklist • Cover sheet • Contact Information • Sponsoring Leader Signature • Clinic Identifiers (i.e., logo, clinic name, clinic city/state) removed from the Body of the Application • Abstract • Organization Profile • Five Narratives (total no more than 20pages) • Supporting Documents (highly recommended if applicable. No more than 15pages) • Submit an Electronic Copy to acclaim@amga.org • Mail Hard Copy to AMGA

  13. Important Dates • June 13, 2014Applications due (5:30 p.m., PDT) • August 22, 2014 Applicants notified of status of their application • November 12-14, 2014 Event honoring Acclaim Award recipient and honorees at AMGA’s Institute for Quality Leadership Conference, New Orleans, LA
